"Utah Marriage Handbook" Request Form
The Utah Marriage Handbook is being distributed by county clerks in each county of the state of Utah when a marriage license is issued. If you are a resident of Utah and want a single free copy sent to you use the request form below. Bulk orders for professional offices can be obtained for Utah residents by prepaying a shipping fee of $8.00 per 100 handbooks. Bulk orders of handbooks can be picked up free of charge by calling 801-526-9317 and scheduling a pick up time.
Non residents of Utah can obtain a copy of the handbook by sending a $3.00 check or money order to cover cost of printing and handling to:
